We DO file to all other insurance companies but are considered out of network.

We do not accept Medicaid, Medicare, Healthy Blue or Home State Health.

We require your patient paperwork, any x-rays and insurance information be provided 24 hours before your appointment or we will need to reschedule your appointment.
